I’m looking to get a tattoo and would like the translation for a quote “
Always on my mind, forever in my heart”

I was thinking more in line of: Memor sum cotidie tui, In animo meo semper remanebis- I am mindful of you everyday, you shall always remain in my "heart".
I think that sounds better.

You don't need a capital in in animo meo, though.

Aswell as a loved one who you are grieving for, would this also be as suitable for addressing a living loved one, from whom you are unable to see for a period of time?

Having in mind a girl or a woman you might say

Infixa es menti meae,
haeres mihi in animo.


You are infixed in my mind,
you adhere to my heart = are firmly impressed upon my heart



but considering a male person: "infixus es menti meae".
