My place of birth

May I request the correct grammar for “my place of birth”, using a derivative of “locu(m/s) nativitatis” please?

Alternatively/additionally, incorporating the place name, eg: “(place name), which is my place of birth”

Do you do need this for a tattoo or for a sentence? In a regular sentence, it would usually be something like ubi natus/-a sim or quo loco natus/-a sim

Just as a sentence, to describe a place; for example: “Bath, (which is) my birthplace.”
Sorry for being confused by the slashes and hyphens, but how would you compose that actual sentence, as a male referring to his birthplace (presumably in the perfect tense?).
